Two people have completely different intrusive thoughts. One is about blasphemy in prayer, the other is about harming their own child. What do they actually have in common?

Not the content. What they share is the second step. Both of them decided the thought said something about who they are, and then did something to settle it. That second step is the part that grows, and it is the same move in both people. It is also why the content of your own thought tells you very little about you.
