Depersonalization-Derealization Disorder (DD) typically manifests as a disruption of body self-awareness. Interoception -defined as the cognitive processing of body signals- has been extensively considered as a key processing for body self-awareness. Recent advances in neuroscience have provided new insights into the understanding of heart-brain interaction and communication. Cardiac information to the brain relies on two pathways, terminating in the insular cortex (IC) and anterior cingulate cortex (ACC), along with the somatosensory cortex (S1-S2). Interoception relying on these neuroanatomical pathways has been shown to modulate social cognition. We report the case study of C.S., a patient with an 'external heart' (an extracorporeal left-univentricular cardiac assist device, LVAD). Comparing cortisol, stress, and sensory sensitivity in children with autism.

Previously we reported that children with autism show significant variability in cortisol. The current investigation was designed to extend these findings by exploring plausible relationships between cortisol and psychological measures of stress and sensory functioning. Salivary cortisol values for diurnal rhythms and response to stress in children with and without autism were compared to parent-report measures of child stress, the Stress Survey Schedule (SSS), sensory functioning, Short Sensory Profile (SSP), and Parenting Stress Index. In autism, a negative relationship between morning cortisol and the SSS revealed that higher observed symptoms of stress were related to lower cortisol. Lower cortisol is seen in conditions of chronic stress and in social situations characterized by unstable social relationships. Media exposure and dimensions of anxiety sensitivity: differential associations with PTSD symptom clusters.

The present investigation examined the impact of anxiety sensitivity (AS) and media exposure on posttraumatic stress disorder (PTSD) symptoms. Reactions from 143 undergraduate students in Hamilton, Ontario were assessed in the Fall of 2003 to gather information on anxiety, media coverage, and PTSD symptoms related to exposure to a remote traumatic event (September 11th). Interoceptive awareness declines with age.

Aging has been shown to increase sensory thresholds for a variety of exteroceptive and proprioceptive stimuli. However, the influence of aging on interoceptive awareness has received relatively little empirical attention. Here we report an inverse association between aging and interoception, as indexed by the ability to sense the heartbeat at rest. In a group of 59 participants ranging in age from 22 to 63 years, age inversely predicted heartbeat detection ability, both within and across several measurement sessions. On average, age accounted for 30% of the variance in heartbeat detection accuracy. Other attribute variables including body mass index and sex were not related to heartbeat detection ability. These findings provide clear empirical evidence that interoception, much like exteroception and proprioception, declines with age.

In two Commentaries, Zimprich et al. (2020), and Ainley et al. (2020) dispute conclusions raised by Zamariola et al. (2018) in a large sample study that questioned the validity of IAcc scores derived from the Heartbeat Counting Task (HCT). After clarifying the reliability of our procedures and the robustness of our main findings, we address the four points of contention discussed in the Commentaries. In doing so, we spell out why research using the HCT faces important interpretational issues, and we call for a theoretical clarification on the construct. In our conclusion, we provide recommendations for improving HCT research and research on interoception in general.

Commentary on "Autism, oxytocin and interoception": Alexithymia, not Autism Spectrum Disorders, is the consequence of interoceptive failure.

In "Autism, oxytocin and interoception" (Neuroscience and Biobehavioral Reviews 47, 410-430) Quattrocki and Friston present their theory of the role of oxytocin in interoception from multiple perspectives. The arguments contained therein are compelling, and highlight the fact that interoception, and the role of oxytocin in interoception, should receive more research attention. However, in addition to outlining the role of oxytocin in interoception the authors also suggest that Autism Spectrum Disorder (ASD) is a result of a failure of this system. It is this latter claim that we disagree with, instead suggesting that alexithymia, rather than autism, is most accurately characterised as a general failure of interoception. A decade of functional brain imaging applied to bladder control.

Over the last 10 years functional brain imaging has emerged as the most powerful technique for studying human brain function. Although the literature is now vast, including studies of every imaginable aspect of cortical function, the number of studies that have been carried out examining brain control of bladder function is relatively limited. Nevertheless those that have been reported have transformed our thinking. This article reviews that development in the context of emerging ideas of interoception and a working model of brain activity during bladder filling and emptying is proposed. Some studies have also been carried out using functional imaging methods to examine pathophysiological bladder conditions or the effect of treatments and these are reviewed and future work anticipated.

The role of interoception and alliesthesia in addiction.

This review presents a novel conceptualization of addiction, integrating the concepts of interoception (i.e., the CNS representation of visceral feelings) and alliesthesia (i.e., that rewarding properties of stimuli are dependent on the internal state of the individual) with existing theories. It is argued that the body state, as defined by the integration of interoceptive information, is a crucial arbiter of the risk for initiation of and transition to compulsive use of addictive compounds. Purpose of the review: Eating in response to negative emotions (EE) may be an explanatory factor of the weight regain of many dieters. This narrative review presents evidence on possible causes of EE and the association of EE with depression and obesity and discusses implications of these findings for the treatment of obesity. Recent findings: Possible causes of EE are high dietary restraint, poor interoceptive awareness, alexithymia, emotion dysregulation and a reversed hypothalamic pituitary adrenal (HPA) stress axis. On the embodiment of emotion regulation: interoceptive awareness facilitates reappraisal.

The ability to cognitively regulate emotional responses to aversive events is essential for mental and physical health. One prerequisite of successful emotion regulation is the awareness of emotional states, which in turn is associated with the awareness of bodily signals [interoceptive awareness (IA)]. This study investigated the neural dynamics of reappraisal of emotional responses in 28 participants who differed with respect to IA. Electroencephalography was used to characterize the time course of emotion regulation. We found that reappraisal was accompanied by reduced arousal and significant modulation of late neural responses. In this Opinion article, we introduce the Embodied Predictive Interoception Coding model, which integrates an anatomical model of corticocortical connections with Bayesian active inference principles, to propose that agranular visceromotor cortices contribute to interoception by issuing interoceptive predictions. We then discuss how disruptions in interoceptive predictions could function as a common vulnerability for mental and physical illness.

The role of interoception in addiction: a critical review.

Interoception refers to a collection of processes by which the state of the body is transmitted back to the brain, giving rise to awareness of the internal milieu, and motivating behavioural responses to homeostatically regulate internal state. Recent work has begun to explore the relevance of this construct to drug addiction: drugs of abuse and drug-related stimuli induce pronounced peripheral changes, and damage to a brain region known to support interoception (the insula) disrupts nicotine dependence. This article critically reviews existing accounts of addiction that suggest impaired interoception contributes to drug abuse. Conceptually, we argue that existing addiction interoception models could be usefully extended by considering (i) the multiple components of the bodily feedback system (signal, perception, and appraisal) and (ii) how individual differences in these three component

The insula and drug addiction: an interoceptive view of pleasure, urges, and decision-making.

We have recently shown that damage to the insula leads to a profound disruption of addiction to cigarette smoking (Naqvi et al., Science 315:531-534, 2007). Yet, there is little understanding of why the insula should play such an important role in an addictive behavior. A broad literature (much of it reviewed in this issue) has addressed the role of the insula in processes related to conscious interoception, emotional experience, and decision-making. Here, we review evidence for the role of the insula in drug addiction, and propose a novel theoretical framework for addiction in which the insula represents the interoceptive effects of drug taking, making this information available to conscious awareness, memory and executive functions. Corporeal awareness is a difficult concept which refers to perception, knowledge and evaluation of one's own body as well as of other bodies. We discuss here some controversies regarding the significance of the concepts of body schema and body image, as variously entertained by different authors, for the understanding of corporeal awareness, and consider some newly proposed alternatives. We describe some recent discoveries of cortical areas specialized for the processing of bodily forms and bodily actions, as revealed by neuroimaging, neurophysiological, and lesion studies. We further describe new empirical and theoretical evidence for the importance of interoception, in addition to exteroception and proprioception, for corporeal awareness, and discuss how itch, a typical interoceptive input, has been wrongly excluded from the classic concept of the proprioceptive-tactile body schema.
