Go with this shirt of mine and cast it upon the face of my father so that he may come to seeing. Joseph said, " Take my shirt to Jacob, for his pain has not ceased from the time he saw the shirt stained with the wolf's blood. The balm will also be my shirt. " When they took the shirt from Egypt, the morning breeze was commanded to take the shirt's scent to Jacob's nostrils so that, before Joseph's messenger could give the good news, he would receive it from the Real's mes- senger and recognize the Real's perfect gentleness and favor toward him. Among the people are some who take peers apart from God, loving them as if loving God. And those who have faith are more intense in love for God. If this were the only verse in the whole Qur'an about the faithful and God's friends, their eminence and honor would be complete, for the Lord of the Worlds is saying, “They love Me intensely and more completely than the unbelievers love their objects of worship.” Do you not see that every once in a while the unbelievers set up another idol and take up another object of worship. Like poor people, they are content with something carved of wood. Then, when they can, they take down the wooden one and make another from silver or gold. If their love for their object of wor- ship is real, why do they turn away from it toward another? They say that a man met a woman recognizer, and her beauty exercised its influence over his heart. Multiple health behaviors: patterns and correlates of diet and exercise in a Hispanic college sample.

Obesity rates are alarming in various ethnocultural groups, particularly in Hispanics. With Hispanics being the fastest growing group to enter college, the aims of the current study were to examine patterns and correlates of exercise and dietary behaviors in Hispanic college students. Data were collected from 693 Hispanic undergraduates who enrolled online and received course extra credit for participation. Individuals completed questionnaires assessing constructs of the transtheoretical model for three health behaviors (exercise, dietary fat, and fruit/vegetable stages of change) along with demographic, psychosocial, and acculturation measures. Less than 1% of students had 0 obesity-relevant risks, while 68% indicated 2 or more risks. Only 2% of the sample met fruit and vegetable guidelines. The role of collective efficacy in exercise adherence: a qualitative study of spousal support and type 2 diabetes management.

Adhering to increased exercise is often reported as one of the greatest challenges facing adults living with diabetes, a perception shared by the married middle-aged and older adults living with diabetes who participated in this study. Understanding how that challenge can best be met is both research and program relevant. Drawing on the social cognitive theory and social support literature, this qualitative study explored the powerful couple relationship in Type 2 diabetes management. The overarching goal of this paper was to illuminate the potentially key role of collective efficacy in exercise adherence in order to develop and test interventions that provide more effective supports for adults living with diabetes. Analyses revealed three core themes used by the couples to describe their perceived beliefs: 'Collective support', 'Collective motivation' and 'Collective responsibility'. Ou

Insulin resistance in children: consensus, perspective, and future directions.

Objective: Emerging data indicate that insulin resistance is common among children and adolescents and is related to cardiometabolic risk, therefore requiring consideration early in life. However, there is still confusion on how to define insulin resistance, how to measure it, what its risk factors are, and whether there are effective strategies to prevent and treat it. A consensus conference was organized in order to clarify these points. Participants: The consensus was internationally supported by all the major scientific societies in pediatric endocrinology and 37 participants. Evidence: An independent and systematic search of the literature was conducted to identify key articles relating to insulin resistance in children. Consensus process: The conference was divided into five themes and working groups: background and definition; methods of measurement and screening; risk factors and

What is the best dose of nature and green exercise for improving mental health? A multi-study analysis.

Green exercise is activity in the presence of nature. Evidence shows it leads to positive short and long-term health outcomes. This multistudy analysis assessed the best regime of dose(s) of acute exposure to green exercise required to improve self-esteem and mood (indicators of mental health). The research used meta-analysis methodology to analyze 10 UK studies involving 1252 participants. Outcomes were identified through a priori subgroup analyses, and dose-responses were assessed for exercise intensity and exposure duration. Other subgroup analyses included gender, age group, starting health status, and type of habitat. The overall effect size for improved self-esteem was d = 0.46 (CI 0.34-0.59, p < 0.00001) and for mood d = 0.54 (CI 0.38-0.69, p < 0.00001). Dose responses for both intensity and duration showed large benefits from short engagements in green exercise, and then diminish

Pharmacological treatment of fibromyalgia syndrome: new developments.

Fibromyalgia is a chronic pain disorder characterized by widespread pain, stiffness, insomnia, fatigue and distress. Several randomized controlled trials (RCTs) have shown moderate effectiveness of pharmacological therapies for fibromyalgia pain. Evidence from these trials suggests that pharmacological therapy can not only improve pain but also fatigue, function and well-being in patients with fibromyalgia. Duloxetine and milnacipran, two highly selective serotonin-norepinephrine (noradrenaline) reuptake inhibitors, and the alpha(2)delta agonist pregabalin have been approved by the US FDA for the treatment of fibromyalgia symptoms. In general, about half of all treated patients seem to experience a 30% reduction of symptoms, suggesting that many patients with fibromyalgia will require additional therapies. Thus, other forms of treatment, including exercise, cognitive behavioural therapie

Recommendations for improved data processing from expired gas analysis indirect calorimetry.

There is currently no universally recommended and accepted method of data processing within the science of indirect calorimetry for either mixing chamber or breath-by-breath systems of expired gas analysis. Exercise physiologists were first surveyed to determine methods used to process oxygen consumption ((.)VO2) data, and current attitudes to data processing within the science of indirect calorimetry. Breath-by-breath datasets obtained from indirect calorimetry during incremental exercise were then used to demonstrate the consequences of commonly used time, breath and digital filter post-acquisition data processing strategies. Assessment of the variability in breath-by-breath data was determined using multiple regression based on the independent variables ventilation (VE), and the expired gas fractions for oxygen and carbon dioxide, FEO2 and FECO2, respectively. 'Knowing what makes them tick': motivating cognitively impaired older adults to participate in restorative care.

Nursing home residents with dementia represent a majority of the most functionally impaired individuals residing in nursing homes. Although many perceive this population as having little restorative potential, maintaining resident functional abilities for as long as possible helps to optimize quality of life and decrease caregiver burden. This study used a qualitative design with a focus group methodology to explore facilitators and barriers to engaging cognitively impaired residents in functional activities and exercise. A purposive sample of seven geriatric nursing assistants who were experts in dementia care participated in the study. Twenty-seven codes were reduced to three themes: (i) knowing what makes them tick and move; (ii) teamwork and utilizing resources; and (iii) barriers to restorative care. The study findings were used to revise the Restorative Care for the Cognitively Imp

Individual, physical and psychological risk factors for neck pain in Australian office workers: a 1-year longitudinal study.

Neck pain is more prevalent in office workers than in the general community. To date, findings from prospective studies that investigated causal relationships between putative risk factors and the onset of neck pain in this population have been limited by high loss to follow-up. The aim of this research was to prospectively evaluate a range of risk factors for neck pain in office workers, using validated and reliable objective measures as well as attain an estimate of 1-year incidence. We assembled a cohort of 53 office workers without neck pain and measured individual, physical, workplace and psychological factors at baseline. We followed participants for 1 year to measure the incidence of neck pain. We achieved 100% participant follow-up. Cox regression analysis was applied to examine the relationship between the putative risk factors and the cumulative incidence of neck pain. The 1-ye

Central and autonomic nervous system interaction is altered by short-term meditation.

Five days of integrative body-mind training (IBMT) improves attention and self-regulation in comparison with the same amount of relaxation training. This paper explores the underlying mechanisms of this finding. We measured the physiological and brain changes at rest before, during, and after 5 days of IBMT and relaxation training. During and after training, the IBMT group showed significantly better physiological reactions in heart rate, respiratory amplitude and rate, and skin conductance response (SCR) than the relaxation control. Differences in heart rate variability (HRV) and EEG power suggested greater involvement of the autonomic nervous system (ANS) in the IBMT group during and after training. Imaging data demonstrated stronger subgenual and adjacent ventral anterior cingulate cortex (ACC) activity in the IBMT group. Frontal midline ACC theta was correlated with high-frequency HR

Predictors of maintaining cognitive function in older adults: the Health ABC study.

Background: Although several risk factors for cognitive decline have been identified, much less is known about factors that predict maintenance of cognitive function in advanced age. Methods: We studied 2,509 well-functioning black and white elders enrolled in a prospective study. Cognitive function was measured using the Modified Mini-Mental State Examination at baseline and years 3, 5, and 8. Random effects models were used to classify participants as cognitive maintainers (cognitive change slope > or = 0), minor decliners (slope < 0 and > 1 SD below mean), or major decliners (slope < or = 1 SD below mean). Logistic regression was used to identify domain-specific factors associated with being a maintainer vs a minor decliner. Results: Over 8 years, 30% of the participants maintained cognitive function, 53% showed minor decline, and 16% had major cognitive decline. In the multivariate m

The obesity paradox, weight loss, and coronary disease.

Purpose: Because obesity is a cardiovascular risk factor but is associated with a more favorable prognosis among cohorts of cardiac patients, we assessed this "obesity paradox" in overweight and obese patients with coronary heart disease enrolled in a cardiac rehabilitation and exercise training (CRET) program, making this assessment in patients classified as overweight/obese using both traditional body mass index (BMI) and percent body fat assessments. Additionally, we assessed the efficacy and safety of purposeful weight loss in overweight and obese coronary patients. Patients and methods: We retrospectively studied 529 consecutive CRET patients following major coronary events before and after CRET, and compared baseline and post program data in 393 overweight and obese patients (body mass index [BMI] > or =25 kg/m(2)) divided by median weight change (median=-1.5%; mean +2% vs -5%, res

Improvement of left ventricular function by lifestyle intervention in obesity: contributions of weight loss and reduced insulin resistance.

Aims/hypothesis: Weight excess and insulin resistance mediate the link between obesity and left ventricular dysfunction. We investigated the effect and mechanisms of lifestyle modification on left ventricular function changes in obese patients. Methods: Reduction of body weight and insulin resistance was sought using a behavioural intervention programme including dietary restrictions and exercise training in 261 patients (age 45 +/- 13 years) with BMI >or=30 kg/m(2), no history of cardiac disease and a normal stress echocardiogram. Each patient underwent echocardiographic measurement of myocardial deformation and velocity at baseline and at 6 month follow-up. Results: Improvements in left ventricular systolic and diastolic function were demonstrated only in patients with significant reduction of weight and/or insulin resistance. Left ventricular improvement was less frequent in patients

Assessing psychological stress among Arab migrant women in the City of Cologne/Germany using the Community Oriented Primary Care (COPC) approach.

Migrants suffer from various health problems including psychological stress. This study is the first to use the Community Oriented Primary Care methodology to address a health problem among Arab migrant women in Germany. In partnership with the Muslim Women's Educational Center in Cologne, we defined a community, identified an important health problem through focus groups, key informant interviews and a community discussion group, and studied psychological stress in a sample of Arab migrant women (N = 116) using a questionnaire that included the Symptom Checklist-90-Revised (SCL-90-R) instrument. Almost 90% of participating women were psychologically stressed. About half of the women cited "feeling lonely" as a main source of stress and another third cited religious, moral and personal factors as sources of stress. While the majority of women reacted to stressful situations with crying,

Exercise and circulating cortisol levels: the intensity threshold effect.

This study examined the influence of exercise intensity upon the cortisol response of the hypothalamic-pituitary-adrenal (HPA) axis. Specifically, we examined exercise at intensities of 40, 60, and 80% maximal oxygen uptake (VO2max) in an attempt to determine the intensity necessary to provoke an increase in circulating cortisol. Twelve active moderately trained men performed 30 min of exercise at intensities of 40, 60, and 80% of their VO2max, as well as a 30-min resting-control session involving no exercise on separate days. Confounding factors such as time of day--circadian rhythms, prior diet--activity patterns, psychological stress, and levels of exercise training were controlled. Cortisol and ACTH were assessed in blood collected immediately before (pre-) and after (post-) each experimental session. Statistical analysis involved repeated measures analysis of variance and Tukey post

The experiences of young people with obesity in secondary school: some implications for the healthy school agenda.

In 1995, the World Health Organisation launched a Global School Health Initiative to reduce health risks among young people. In the UK, the National Healthy School Programme (HSP) developed as part of a wider government commitment to promoting social inclusion. One of the key issues to be tackled by the programme is childhood obesity, for obesity is widely argued to be a public health problem for which a solution needs to be found. However, the assumption that obesity is necessarily problematic and that a higher body weight leads to health problems and social exclusion, is not without challenge. Critics of anti-obesity campaigns question the significance of weight per se and highlight the potential implications of discrediting not only fatness, but also the people who are seen as fat. This paper therefore explores the experiences of young people with obesity within the secondary school e

Exercise training improves the antioxidant enzyme activity with no changes of telomere length.

The purpose of this study was to determine the changes of both oxidant and antioxidant levels with exercise training in obese middle-aged women. The association between telomere length and oxidative stress with exercise was also examined. Sixteen obese middle-aged women participated in this study. The subjects were randomly divided into exercise group (EX) and control group (CON). EX performed aerobic exercise training for 6 months. DNA was extracted from leukocytes in peripheral blood and their telomere lengths were measured by real time PCR analysis. Long-term exercise training decreased body weight and BMI, and increased VO2 max. Resting levels of erythrocyte glutathione peroxidase activity were higher in EX compared to CON. Superoxide dismutase (SOD) activities were higher after the acute exercise test at mid-intensity in post-exercise training than in the pre-exercise training condi

Development of a PC-based diabetes simulator in collaboration with teenagers with type 1 diabetes.

Background: The main aim of this study was to develop and test in a pilot study a PC-based interactive diabetes simulator prototype as a part of future Internet-based support systems for young teenagers and their families. A second aim was to gain experience in user-centered design (UCD) methods applied to such subjects. Methods: Using UCD methods, a computer scientist participated in iterative user group sessions involving teenagers with Type 1 diabetes 13-17 years old and parents. Input was transformed into a requirements specification by the computer scientist and advisors. This was followed by gradual prototype development based on a previously developed mathematical core. Individual test sessions were followed by a pilot study with five subjects testing a prototype. Modulation of emotional appraisal by false physiological feedback during fMRI.

Background: James and Lange proposed that emotions are the perception of physiological reactions. Two-level theories of emotion extend this model to suggest that cognitive interpretations of physiological changes shape self-reported emotions. Correspondingly false physiological feedback of evoked or tonic bodily responses can alter emotional attributions. Moreover, anxiety states are proposed to arise from detection of mismatch between actual and anticipated states of physiological arousal. However, the neural underpinnings of these phenomena previously have not been examined. Methodology/principal findings: We undertook a functional brain imaging (fMRI) experiment to investigate how both primary and second-order levels of physiological (viscerosensory) representation impact on the processing of external emotional cues. 12 participants were scanned while judging face stimuli during both

Interoceptive sensitivity and physical effort: implications for the self-control of physical load in everyday life.

This study examined the relationship between interoceptive sensitivity and the behavioral self-regulation of physical load. According to their performance in a heartbeat detection task, 34 participants were classified as good or poor heartbeat perceivers. Participants pedaled on a bicycle ergometer for 15 min and were free to choose the tempo of their cycling. Good heartbeat perceivers covered a significantly shorter distance and showed a significantly smaller increase in mean heart rate, stroke volume, and cardiac output. There were significant negative correlations between heartbeat perception score and covered distance, changes in heart rate, changes in stroke volume, and changes in cardiac output. These results were not explained by differences in physical fitness level (PWC150). The findings suggest that good heartbeat perceivers show a more finely tuned behavioral self-regulation o
