With a bedtime dose, the clock is the dose. Give it too early and the medicine switches on in the small hours, which buys you a child awake at half past four and no appetite at breakfast.

The ordinary effects still apply: appetite tends to dip around the middle of the day and sleep can be harder to come by. What changes is that one decision, the hour it is taken, now sets when everything starts. Agree a time with the prescriber, write it somewhere visible, and hold it steady for a fortnight before judging anything, because moving it about makes the pattern impossible to read.

What the research says

Stimulant effects on the body are usually modest and still measurable, which is an argument for monitoring rather than assuming. A meta analysis of adults taking stimulants found small but consistent rises in heart rate and blood pressure, enough to be worth checking rather than ignoring. That work is about the heart rather than about sleep or dose timing, so read it as a general case for keeping an eye on things.
