The message is a key rather than a parcel. It fits a lock on the receiving cell, turns it, and then comes away again instead of being swallowed.
This one detail makes the rest make sense. Because the message comes back off, it can be collected and used again, which is what a reuptake inhibitor is interfering with. It is also why a drug that sits in a lock can block a message without destroying anything. If you have been picturing chemicals being absorbed and used up, most explanations of these medicines will keep sounding wrong.

What the research says
Knowing the mechanism does not yet tell you how one person will respond. A systematic review of pharmacogenetics in adults found genetic variants explained some of the variation in response to methylphenidate, but not enough to guide prescribing in practice. Some and not enough are both accurate there. Understanding the machinery and predicting an individual remain two different achievements.

A child who has spent years being told they are careless will often say yes to whoever finally seems glad of them. Work on how they see themselves is not a soft extra to the clinical work.
If sleep has gone since starting, look at what time the dose is being taken before deciding the medicine is the problem. A long acting dose swallowed at one in the afternoon is still working at bedtime.
These are among the best studied medicines in psychiatry, in use for decades. Most of the hesitancy around them traces back to how they get covered in the press rather than to the safety record.
