One kind of long acting tablet works like a tiny pump. Water is drawn in, an inner layer swells, and the medicine is pushed out steadily through a laser drilled hole. The empty shell comes out whole in the stool, which alarms anybody who was not warned.
The design gives a level that climbs gently through the day instead of peaking early, which helps because the body becomes a little less responsive to a steady level over those same hours. Do mention the shell in advance. Otherwise the first sighting gets read as the tablet having passed through unused, and a treatment that was working gets abandoned over an empty husk.

What the research says
There is a decent mechanistic account of what these drugs are doing. A systematic review and meta analysis of imaging studies found stimulants consistently normalised activity in particular frontal and striatal regions during tasks that require inhibition and attention. Those were short tasks done in scanners rather than measures of a working day, so they explain the direction of the effect rather than proving what any given tablet delivers at four in the afternoon.

An untreated attention difficulty rarely stays a blank space. People tend to find something that steadies them, and what they find is not always kind.
For someone who cannot swallow tablets, or whose mornings fall apart long before the tablet gets taken, patches exist that release medicine slowly through the skin over hours.
A few medicines leave very little space between the amount that helps and the amount that harms. Lithium is the classic one, and with those the blood test is part of the treatment rather than paperwork.
