When something has become impossible, start far below it. Sitting in the parked car with the engine off is a real first step, and it is the one that gets somewhere.
Choose a version so small it feels slightly ridiculous, repeat it until it is dull, then add one notch. Sit in the car. Then start the engine. Then to the end of the road and back. Boredom is the signal to move up, not confidence, which tends to arrive later than people expect. If a step brings the fear back hard, drop down one instead of pushing through, because you are gathering evidence rather than proving a point today.
Qur’an and Sunnah
Do not rush to recite before the revelation is fully complete (Quran 20:114). Going at the pace the thing actually allows is presented as right, not as timidity.

What the research says
A meta-analytic review pooled nine randomised and three uncontrolled trials to ask whether the gains from psychological treatment for adults with ADHD are still there at longer follow-up. None of it tests a ladder of small steps in particular. It is here as the honest ceiling on advice of this kind: structured psychological work has support behind it, and how well it holds over time is still being worked out.

Strength of evidence: one line of research. A single study, or a theoretical account.
Why it works
Fear settles when you stay somewhere long enough for nothing bad to happen, and that only works if the step is small enough to stay in.
When not to
If someone cannot leave the house at all, or is talking about not wanting to be here, this needs a proper assessment before any ladder is built.

Near this one
Sort the work into hardest, middling and easiest, then choose the order on purpose. Easy first for momentum or hard first while you are sharp, both are fine. What is not fine is letting the most interesting thing win by default.
Saying no accurately depends on a calendar you actually keep. You cannot check whether Thursday is free if Thursday looks blank when it is not.
Keep both tools, and settle in advance where each thing goes. The deciding is what wears you out, so make that decision once instead of forty times a week.
