Optimizing inhibitory learning during exposure therapy.
Prevailing models of exposure therapy for phobias and anxiety disorders construe level of fear throughout exposure trials as an index of corrective learning. However, the evidence, reviewed herein, indicates that neither the degree by which fear reduces nor the ending fear level predict therapeutic outcome. Developments in the theory and science of fear extinction, and learning and memory, indicate that 'performance during training' is not commensurate with learning at the process level. Inhibitory learning is recognized as being central to extinction and access to secondary inhibitory associations is subject to influences such as context and time, rather than fear during extinction training. Strategies for enhancing inhibitory learning, and its retrieval over time and context, are reviewed along with their clinical implications for exposure therapy and directions for future research.

Prefrontal involvement in the regulation of emotion: convergence of rat and human studies.
Emotion regulation is a process by which we control when and where emotions are expressed. Paradigms used to study the regulation of emotion in humans examine controlled responses to emotional stimuli and/or the inhibition of emotional influences on subsequent behavior. These processes of regulation of emotion trigger activation of the ventromedial prefrontal cortex and inhibition of the amygdala. A similar pattern of activation is seen in rodents during recall of fear extinction, an example of emotional regulation. The overlap in circuitry is consistent with a common mechanism, and points toward future experiments designed to bridge human and rodent models of emotion regulation.

The brain's ability to associate different stimuli is vital for long-term memory, but how neural ensembles encode associative memories is unknown. Although visual avoidance of faces is a hallmark feature of social anxiety disorder (SAD) on clinical and theoretical grounds, empirical support is equivocal. This review aims to clarify under which conditions socially anxious individuals display visual avoidance of faces. Through a systematic search in Web of Science and PubMed up to March 2019 we identified 61 publications that met the inclusion criteria. Effects of D-cycloserine on extinction: translation from preclinical to clinical work.
Administration of benzodiazepines or serotonin reuptake inhibitors in combination with behavior therapy for the treatment of many anxiety disorders has generally lead to only modest gains. In this article we suggest that pharmacotherapy aimed not at treating the symptoms of anxiety but instead aimed at improving the learning that takes place in exposure therapy might actually improve the effectiveness of exposure therapy. Intranasal oxytocin modulates brain activity during emotional processing in children with treatment resistant conduct problems.
One of the most highly replicated neural correlates of Conduct Problems (CP) is amygdala hypoactivity to another person's fear. We recently reported that this correlate was only observed in boys with persistent CP (i.e. antisocial behaviour that persisted following a gold-standard psychological intervention), suggesting that amygdala hypoactivity to fear could be an important neural signature for treatment-resistant CP, and a putative target for future treatments. Potential treatment candidates include the oxytocin system, as this has been reported to modulate amygdala activity and social behaviour across species.
