A double-blind comparison of sexual functioning, antidepressant efficacy, and tolerability between agomelatine and venlafaxine XR.
Impaired sexual function is associated with major depressive disorder in the untreated state and is often more prevalent during antidepressant therapy, which frequently results in poor treatment compliance. The advent of non-invasive functional brain imaging has clarified which regions of the brain are recruited during sexual arousal. Injuries to those regions, and to the spinal cord and peripheral nerves that link genitalia to limbic and cognitive centres, can profoundly influence sexual wellbeing. In epilepsy, expressions of hypersexuality and hyposexuality interact with the location of epileptogenic foci in the temporolimbic circuitry, and are tempered by the sexual effects of drug treatments. We outline the sexual consequences of epilepsy, stroke, multiple sclerosis, Parkinson's disease, and other common neurological disorders. Management of sexual dysfunction from both disease and treatment is discussed. Nerve-sparing techniques could mitigate the substantial sexual dysfunction in both men and women through surgical disruption of the autonomic nerves during radical pelvic surgery.

Gender-transformative interventions to reduce HIV risks and violence with heterosexually-active men: a review of the global evidence.
Emerging out of increased attention to gender equality within HIV and violence prevention programming has been an intensified focus on masculinities. A new generation of health interventions has attempted to shift norms of masculinity to be more gender equitable and has been termed "gender-transformative." We carried out a systematic review of gender-transformative HIV and violence prevention programs with heterosexually-active men in order to assess the efficacy of this programming. After reviewing over 2,500 abstracts in a systematic search, a total of 15 articles matched review criteria. The evidence suggests that gender-transformative interventions can increase protective sexual behaviors, prevent partner violence, modify inequitable attitudes, and reduce STI/HIV, though further trials are warranted, particularly in establishing STI/HIV impacts. Parents can significantly impact their adolescent child's sexual and relationship decision-making, yet many parents are not effectively communicating with their teens about these topics. Media are sexual socialization agents for adolescents, which can encourage early or risky sexual activity. Media Aware Parent is a web-based program for parents of adolescents that was designed to improve adolescent sexual health by providing parents with the skills to have high-quality communication with their child about sex and relationships as well as to mediate their media usage. Risk factors for sexual dysfunction after rectal cancer treatment.
This study aimed to identify risk factors for long-term sexual dysfunction (SD) after rectal cancer treatment. Patients with resectable rectal cancer were randomised to total mesorectal excision with or without preoperative radiotherapy (PRT). Preoperatively and at 3, 6, 12, 18 and 24 months postoperatively, SD scores were filled out in questionnaires. Possible risk factors for postoperative deterioration of sexual functioning, including patients' demographics, tumour-specific factors and treatment-related variables, were investigated with univariate and multivariable regression analyses. Increase in general SD, erectile dysfunction and ejaculatory problems were reported by 76.4, 79.8 and 72.2 percent of the male patients, respectively. Combined brief psychosexual intervention after mastectomy: effects on sexuality, body image, and psychological well-being.
Background: Mastectomy poses a severe threat to body image and sexuality, interfering with the psychological well-being of both the patient and her partner. Although many psychotherapeutic approaches have been assessed and found effective in such patients, few studies have assessed couple or sex therapy in mastectomy patients. Methods: We report on a structured combination of brief couples and sex therapy (CBPI) used in 20 women with in situ breast cancer and mastectomy and their partners. An equal number of mastectomy patients were used as controls. Results: CBPI patients showed significant improvement in depression and state anxiety scores, as well as in body image, satisfaction with relationship, presumed attractiveness to their partner, orgasm frequency and communicating their desire. An observational longitudinal study to evaluate miction, defecation, and sexual function after radical hysterectomy with pelvic lymphadenectomy for early-stage cervical cancer.
The objective of this study was to evaluate the problems with miction, defecation, and sexuality after a radical hysterectomy with or without adjuvant radiotherapy for the treatment of cervical cancer stage I-IIA. This study included an observational longitudinal study of self-reported bladder, defecation, and sexual problems with a baseline score. Ninety-four women were included in the study. An age-matched control group consisted of 224 women. The patients showed significantly more negative effects on sexual function compared with both the controls and their situation before the treatment throughout 24 months of follow-up. The problems included less lubrication, a narrow and short vagina, senseless areas around the labia, dyspareunia, and sexual dissatisfaction. Reducing problematic pornography use with imaginal retraining-A randomized controlled trial.
Background and aim: Problematic pornography use (PPU) can be a manifestation of compulsive sexual behavior disorder (CSBD). Studies investigating PPU confirm approach-avoidance tendencies in response to pornographic stimuli in this population. This study show indications of the significance of the efficacy of imaginal retraining, a variant of approach bias modification, as an intervention for PPU. A review of couple-based interventions for enhancing women's sexual adjustment and body image after cancer.
Despite the growing interest in couple-based interventions for women with cancer, only few of these interventions primarily focus on improving women's sexual and body image. A review of studies that investigated the impact of an intervention on women's sexual adjustment and/or body image was conducted. Twelve studies met inclusion criteria for this review: 8 randomized control trials, 2 pilot randomized control trials, and 2 quasi-experimental designs. Our analysis revealed that interventions that produce stronger effects tend to be couple-focused and include treatment components that (1) educate both partners about the woman's diagnosis and treatments, (2) promote couples' mutual coping and support processes, and (3) include specific sexual therapy techniques to address sexual and body image concerns. Sexual communication mediates cognitive-behavioral couple therapy outcomes: A randomized clinical trial for provoked vestibulodynia.

A randomised controlled trial of a couples-based sexuality intervention for men with localised prostate cancer and their female partners.
Objective: The diagnosis and treatment of prostate cancer is followed by substantive sexual morbidity. The optimal approach for intervening remains unclear. Methods/design: A three-arm randomised control trial was undertaken with 189 heterosexual couples where the man had been diagnosed with prostate cancer and treated surgically. The efficacy of peer-delivered telephone support versus nurse-delivered telephone counselling versus usual care in improving both men's and women's sexual adjustment was investigated. Assessments were undertaken at baseline (pre-test) with follow-up at 3, 6 and 12 months. Group mindfulness-based therapy significantly improves sexual desire in women.
At least a third of women across reproductive ages experience low sexual desire and impaired arousal. There is increasing evidence that mindfulness, defined as non-judgmental present moment awareness, may improve women's sexual functioning. The goal of this study was to test the effectiveness of mindfulness-based therapy, either immediately or after a 3-month waiting period, in women seeking treatment for low sexual desire and arousal.
