If someone was adopted, or simply does not know their family history, say early and plainly that this is not a problem. It helps to have and it is not needed.
Otherwise the question lands as one more reminder of what is missing, and some people will spend the rest of the appointment feeling they brought the wrong thing. A sentence at the start settles it: family history is useful where it exists, and plenty of assessments are made without it. Then move on instead of circling back.

What the research says
A population based birth cohort found many impaired young adults with ADHD symptoms had no documented childhood onset disorder, which unsettles the assumption that a valid diagnosis always requires clear documented history. Missing history is common and workable. It is a single cohort, so take it as a reason to look carefully rather than as licence to skip history taking.

Ask whether anyone kept the old school reports. The same lines turn up generation after generation: not working to potential, talks too much, never has the right books with him.
There is no box you type 'always late' into and get the answer out of. When a sensible strategy is not landing, the work is finding out what is blocking it rather than producing another strategy.
Alongside diagnoses, ask how relatives died. Falls from roofs, crashes, deaths with the safety gear left on the ground: these recur in some family lines and they say something a list of diagnoses cannot.
