Think of it the way you would think about diabetes. Something to manage with several things at once, most days, for as long as it is there.

That frame does a lot of quiet work. It says the treatment is ongoing rather than a course you finish, it explains why things slide back when treatment stops, and it makes regular review normal instead of a sign that something has gone wrong. Most usefully, it takes the blame out of a relapse. Nobody says a person failed at diabetes when their blood sugar rises after they stop their medicine.

Qur’an and Sunnah

Anyone who is ill or on a journey should make up for the lost days by fasting on other days later (Quran 2:185). A lasting difficulty is met with a standing arrangement rather than with blame.

شَهۡرُ رَمَضَانَ ٱلَّذِيٓ أُنزِلَ فِيهِ ٱلۡقُرۡءَانُ هُدࣰ ى لِّلنَّاسِ وَبَيِّنَٰتࣲ مِّنَ ٱلۡهُدَىٰ وَٱلۡفُرۡقَانِۚ فَمَن شَهِدَ مِنكُمُ ٱلشَّهۡرَ فَلۡيَصُمۡهُۖ وَمَن كَانَ مَرِيضًا أَوۡ عَلَىٰ سَفَرࣲ فَعِدَّةࣱ مِّنۡ أَيَّامٍ أُخَرَۗ يُرِيدُ ٱللَّهُ بِكُمُ ٱلۡيُسۡرَ وَلَا يُرِيدُ بِكُمُ ٱلۡعُسۡرَ وَلِتُكۡمِلُواْ ٱلۡعِدَّةَ وَلِتُكَبِّرُواْ ٱللَّهَ عَلَىٰ مَا هَدَىٰكُمۡ وَلَعَلَّكُمۡ تَشۡكُرُونَ

It was in the month of Ramadan that the Quran was revealed as guidance for mankind, clear messages giving guidance and distinguishing between right and wrong. So any one of you who is present that month should fast, and anyone who is ill or on a journey should make up for the lost days by fasting on other days later. God wants ease for you, not hardship. He wants you to complete the prescribed period and to glorify Him for having guided you, so that you may be thankful

What the research says

A fifty two week open label study followed adults taking extended release methylphenidate who had already finished a shorter placebo controlled trial, to see how well the benefit held up over time. Studies of that length exist because the honest question in a lasting condition is about the long run. It was open label, with everybody knowing what they were taking, so it carries less weight than the controlled trial before it.

Strength of evidence: one line of research. A single study, or a theoretical account.

Why it works

A condition that stays needs treatment that stays, so a return of symptoms after stopping is information rather than failure.
