Before anyone concludes that medication does not work for you, check what dose was actually reached. Doses in general practice often stay lower than a specialist would use, and a fair share of so called non responders respond once the dose goes up.

You are allowed to ask this, gently and without any accusation in it: what was the highest dose tried, and how long did it stay there? A trial that stopped at the starting dose has not really tested anything. If the answer is that it never went up, asking for a review, or for a referral to someone who does this all day, is a reasonable next move.

What the research says

When methylphenidate treatment is delivered properly, the gains can hold. A follow up analysis of the COMPAS trial in adults found that improvements from methylphenidate, and from psychotherapy, were largely maintained eighteen months after the trial ended. That is a study of what lasts rather than a verdict on dosing, but it shows what an adequate trial can be worth, which is what a half finished one throws away.
