Wanting to try everything else first is a normal instinct and worth taking seriously. It is still fair to ask why we would have a child squint for a year before trying glasses.

The cost of waiting is not nothing. A school year is a school year, friendships form or fail to, and a child's sense of who they are is being built the whole time. There is a practical catch too: the very difficulties you would be treating are the ones that make routines, coaching and reward charts so hard to keep going. Nobody needs to be rushed, and the delay deserves the same scrutiny as the medicine.

Before you use this

This explains how treatments work. Never start, stop or change a medication because of something you read here.

Qur’an and Sunnah

'He who cures me when I am ill' (Quran 26:80). Abraham places the seeking of a cure inside his faith rather than against it.

وَإِذَا مَرِضۡتُ فَهُوَ يَشۡفِينِ

He who cures me when I am ill

What the research says

An updated review of treatment for adolescents found methylphenidate well established while behaviour therapy had only preliminary support in that age group. That is a statement about how strong the evidence is for each, not about what any one family ought to do. It does mean the two options are not on equal footing when you are deciding what to try first.

Sibley MH, Kuriyan AB, Evans SW, Waxmonsky JG, Smith BH. (2014). Pharmacological and psychosocial treatments for adolescents with ADHD: an updated systematic review of the literature.. Clinical psychology review doi

Strength of evidence: one line of research. A single study, or a theoretical account.

Why it works

Time spent waiting is not neutral, because the difficulty carries on doing its work meanwhile.

When not to

None of this replaces a proper assessment, which comes before any decision about medication.

This one is still a draft. It is waiting on a scholar and a clinician to read it before it is published.
