Holding off on treatment can feel like the safe, neutral option. For a child who is struggling now, it is a choice with its own costs rather than an absence of one.

It is completely reasonable to want to try gentler things first, and many of them are worth trying. What is worth resisting is the sense that doing nothing keeps your hands clean. The months pass either way and the child lives through them either way. Put the cost of waiting on the table beside the cost of treating, and then decide.

Before you use this

This explains how treatments work. Never start, stop or change a medication because of something you read here.

Qur’an and Sunnah

'do not forbid the good things God has made lawful to you' (Quran 5:87). Ruling out a permitted remedy on principle is its own decision, and it deserves the same weighing as taking it.

You who believe, do not forbid the good things God has made lawful to you- do not exceed the limits: God does not love those who exceed the limits

What the research says

A network meta-analysis of randomised trials in children and adolescents found stimulant medication gave the largest symptom reductions of any single intervention, with the non-drug options performing less well on the same measures. That compares symptom scores over the length of trials rather than passing a verdict on a childhood. It does mean the two paths are not equal in what they have been shown to do.

Strength of evidence: one line of research. A single study, or a theoretical account.

Why it works

Both roads have a price, and only one of them usually gets counted.

When not to

A family who have weighed this carefully and chosen differently are not doing something wrong, and the point here is only that the waiting gets weighed too.

This one is still a draft. It is waiting on a scholar and a clinician to read it before it is published.
