A synapse is three things rather than one: the end of the cell sending, the gap itself, and the surface of the cell receiving. Ask which of the three a drug acts on and most of its behaviour follows.
Drugs that block the clearing away work on the sending side. Drugs that imitate or block the message work on the receiving side. Drugs that stop the message being broken down work in and around the gap. When somebody explains a new medicine to you, that single question sorts it faster than the brand name will.

What the research says
Different points in the chain can arrive at a similar place. A review of atomoxetine in children and adolescents found it more effective than placebo, generally well tolerated, and not inferior to immediate release methylphenidate in the trials it gathered, despite acting on a different transporter. Not inferior is a modest claim rather than a strong one. It does show that the site of action is not the whole story about the result.

A long acting injection into muscle releases its medicine slowly over weeks. It lifts the daily burden of remembering, at the price of not being able to stop quickly if the medicine disagrees with you.
A nerve cell spends its life adding up. Signals arrive along its branches, the body of the cell totals them, and at one particular point it either fires or it does not.
Doing the behavioural work alongside a medicine can mean managing on less of it. That matters most for the side effects that follow the size of the dose.
