A tablet goes into your bloodstream and reaches everywhere the chemical it acts on is used, and these chemicals are used in far more than the circuit anyone is aiming at. Sleep, gut, appetite, heart rate: none of them are the target and all of them can be touched. It helps to hear that a side effect is usually the same mechanism doing its job somewhere you did not want it, rather than the drug going wrong.

What the research says

The heart is a simple case. A meta-analysis of adults on stimulant treatment found small but consistent rises in heart rate and blood pressure. Small and consistent is the honest description of it: not a reason for alarm, and a reason for somebody to keep an eye on it.
