Areas › Focus, starting, and ADHD › Medication and how it works
Long acting stimulants are the usual starting point for a plain reason: they cover the most hours and they remove the lunchtime dose, which is the dose ADHD is most likely to swallow whole.
Remembering a tablet in the middle of a busy day is itself an executive task, so asking for it is asking the person to do the thing they came for help with. Some people still add a small short acting dose in the late afternoon to reach into the evening. That is a normal adjustment to raise with a prescriber rather than proof that the long acting one has failed.
This explains how treatments work. Never start, stop or change a medication because of something you read here.
Qur’an and Sunnah
'God does not burden any soul with more than it can bear' (Quran 2:286). A regime that leans on a capacity someone does not have is a burden to remove rather than a test to pass.
God does not burden any soul with more than it can bear: each gains whatever good it has done, and suffers its bad- ‘ Lord, do not take us to task if we forget or make mistakes. Lord, do not burden us as You burdened those before us. Lord, do not burden us with more than we have strength to bear. Pardon us, forgive us, and have mercy on us. You are our Protector, so help us against the disbelievers.’

What the research says
This was tested rather than assumed. A randomised crossover trial compared once daily long acting methylphenidate against three times daily dosing in children, in the laboratory and in ordinary settings, and the once daily form matched the split schedule. The trial is old and modest in size, and it answered the practical question it set out to answer: the convenience did not cost effectiveness.

Strength of evidence: one line of research. A single study, or a theoretical account.
Why it works
A medicine only works on the days it is actually taken, so fewer daily steps means more working days.
This one is still a draft. It is waiting on a scholar and a clinician to read it before it is published.
Near this one
Where anxiety sits alongside the attention difficulty, a non-stimulant is sometimes the better first choice. Stimulants sharpen anxiety in some people, and that is a hard trade to live with.
The growth effect is usually small: something like a couple of pounds and a centimetre or two less than expected in the first year or two, with most children catching up afterwards. Knowing the size of it helps, because both blank reassurance and quiet dread are worse than the actual number.
One question is worth asking before choosing between products: which part of the day actually goes wrong? Bead type capsules tend to give more early on, while the pump type keeps more back for the afternoon.
