Areas › Focus, starting, and ADHD › Medication and how it works
A prescribed dose swallowed as a tablet is set to bring you into the ordinary range, not to make you high. The rise it produces is too slow and too small to be the sort of thing anybody chases.
Misuse in the research is concentrated among people who do not have ADHD and who take the medicine in ways it was never designed for, by crushing it, snorting it, or taking several at once. That changes how fast it arrives, and speed of arrival is the part that makes anything addictive. If you have wondered whether needing it daily makes you dependent, the more useful questions are whether you are taking it as prescribed and whether it is helping.
This explains how treatments work. Never start, stop or change a medication because of something you read here.
Qur’an and Sunnah
Who has forbidden the adornment and the nourishment God has provided for His servants (Quran 7:32). Denying yourself something permitted and needed is questioned there rather than admired.
Say [Prophet], ‘Who has forbidden the adornment and the nourishment God has provided for His servants?’ Say, ‘They are [allowed] for those who believe during the life of this world: they will be theirs alone on the Day of Resurrection.’ This is how We make Our revelation clear for those who understand

What the research says
A long term follow up using data from the main childhood ADHD trial, with methods designed to handle confounding, found stimulant treatment was not associated with increased later substance use. Separately, a critical review of brief adult screening questionnaires warns they produce many false positives, so a positive screen should lead to fuller assessment rather than straight to a prescription. Both matter here: the treatment does not look like a gateway, and getting the diagnosis right is part of why.

Strength of evidence: one line of research. A single study, or a theoretical account.
Why it works
It is the speed of the rise that makes a substance rewarding, and a tablet swallowed whole rises slowly.
When not to
If you find yourself taking more than prescribed, or taking it for reasons other than the ones it was prescribed for, tell the prescriber plainly and early.
This one is still a draft. It is waiting on a scholar and a clinician to read it before it is published.
Near this one
One of these medicines arrives inactive and has to be broken down by your own body before it does anything at all. That design flattens the rise and removes the spike that misuse depends on.
It is a fair question to ask of anybody telling you about a medicine: what is their interest in it. Good clinicians answer without flinching, and some have said the unwelcome thing to a room that paid for their flight.
Prescribers usually begin below the dose likely to work and build upwards. That means the first weeks can be underwhelming by design, and it is not the moment to decide the whole thing has failed.
