With many of these medicines the unpleasant part turns up on day one and the good part takes weeks. Knowing that in advance is most of what carries people across the gap.

The first fortnight can feel like a bad bargain, because it is one: you pay now for something that arrives later. Put a date in the diary two to four weeks out and agree with your prescriber that you will judge it then. Write down how you feel at the start as well, since it is surprisingly hard to remember by the time things have improved.

What the research says

A clinical review of atomoxetine looked at when its benefit appears and found it builds gradually across weeks rather than showing up within hours, unlike the stimulants. That is one drug in one condition, and the pattern is a familiar one across psychiatric medicines. It is the reason a fair trial has a length as well as a dose.

Why it works

The body reacts to the drug at once and only adapts to it slowly, so the costs come before the benefits.

Anything severe, and any thought of harming yourself, is not something to sit out until the review date.
