Half the useful health information arrives sideways. Someone says 'I am so tired' on the way into another subject, and that is an opening worth taking gently.
People rarely bring sleep or food to a session as a topic, because they do not file them as symptoms. So the tiredness, the skipped meals and the fourth coffee turn up as asides. Pick one of them up, ask a plain question about it, and put it on the agenda if it turns out to matter. One thread at a time is plenty.

Qur’an and Sunnah
Job 'cried to his Lord, suffering has truly afflicted me' (Quran 21:83), without pretending to be well. Plain statements of how things really are deserve hearing, even when they slip out in passing.
۞وَأَيُّوبَ إِذۡ نَادَىٰ رَبَّهُۥٓ أَنِّي مَسَّنِيَ ٱلضُّرُّ وَأَنتَ أَرۡحَمُ ٱلرَّٰحِمِينَ

What the research says
Adding structured psychological work alongside medication has been tested properly. A randomised controlled trial in adults already taking medication for ADHD found that adding cognitive behavioural therapy improved core symptoms, with mixed effects on the other problems they were carrying. It tested a full course of therapy rather than a conversational habit, so it supports the broader claim that the therapy hour adds something the prescription does not.

Why it works
Things nobody counts as a symptom never get raised on purpose, so they have to be caught in passing.

Where anxiety sits alongside the attention difficulty, a non-stimulant is sometimes the better first choice. Stimulants sharpen anxiety in some people, and that is a hard trade to live with.
There are really only two directions in the nervous system, go and stop. Almost everything a medicine does comes down to shifting the balance between them somewhere.
Wanting to try everything else first is a normal instinct and worth taking seriously. It is still fair to ask why we would have a child squint for a year before trying glasses.
